If you live with headaches, you may be interested in taking this quiz. You can review the type of pain, where the pain is, symptoms, and so on.
Upon completion, you print it out and bring it to your healthcare practitioner. This questionnaire is useful because it can help you describe to your doctor the type of pain you are experiencing. There is one page where you are encouraged to enter your personal information, but you don’t need to do so; you can just click on the “continue” button and the questionnaire will bring you to the final pages.
