If you have a child with sickle cell, keeping track of painful periods, doctors’ visits, and medications may be confusing sometimes.
The Pediatric Pain Sourcebook has this PDF document available for you that might help you organize things. The first page has the Wong-Baker pain scale, for younger children to be able to show what type of pain they’re feeling. The remaining pages contain information such as when to call your doctor, but also information for you to fill out for your regular or emergency doctor visits.
Besides the usual identifying information, you also can enter information on immunizations, other medical issues, the pain management protocol used for your child, and a sickle cell visit log.
