Wrist pain and carpal tunnel questionnaire

Another in the series of questionnaires I found for Help My Hurt visitors. This questionnaire is for adults who have hand or wrist pain.

Sometimes, although we can be living with a specific type of pain for a while, it can be hard to explain to a doctor or dentist where the pain is and how it feels.

This questionnaire helps you describe the type of pain you have. There’s a section where you can outline where the pain is and describe the type of pain or numbness. You can indicate where the worst of the pain is located. Upon completion, you print it out and bring it to your healthcare practitioner. There is one page where you are encouraged to enter your personal information, but you don’t need to do so; you can just click on the "continue" button and the questionnaire will bring you to the final pages.
